RO # 1191 State Farm Claim # 38-K625-35901 2000 Pontiac Sunfire March 4, 2005 Original Estimate w/ T.J. Kehoe Rick wrote for rear suspension beam, due to the fact that vehicle’s right rear wheel was in at the bottom due to the accident. No other suspension mount points control the way the wheel is positioned. TJ denied the replacement of the rear beam and wrote for a new suspension strut and wheel. March 11, 2005 Cosmetic repairs completed to vehicle and suspension components that were written for were installed. This did not fix the problem of the wheel being in at the bottom. A supplement request was issued to State Farm. March 16, 2005 T.J. wrote supplement for a used rear suspension beam and did not include the transfer (overhaul) of the original components to the used assembly. This could have created a major safety issue. He also failed to write for an alignment check for the rear suspension beam. Any used suspension pieces need to be checked for trueness (who knows what the vehicle that they came from has been through). March 16, 2005 Supp request was added to transfer rear suspension beam components, bleed breaks and check rear alignment. This supplement was later approved without the alignment check. March 23, 2005 Installed rear suspension beam and performed an alignment check for safety. Rear suspension required shim kits for both sides. Documented before and after measurements of alignment and installed shims. Put in final supplement request along with copy’s of alignment specs and invoices. Repairs completed today. March 25, 2005 T.J. later denied supplement due to us having performed the work before it was approved. Reason why work was performed was because we could not afford to have alignment rack be tied up for this period of time. Rick proceeded to call Ron Constagleola, Claims Supervisor @ State Farm and faxed over all of the paperwork April 01, 2005 Ron approved supplement and vehicle could be released. Customers 30 day rental had expired. 14 Days wasted on waiting on supplements for an item that should have been written from the original estimate.
